In 2017, Quartz partnered with several major insurers in the area to create a comprehensive local health solution offering multiple health insurance plan types, hence the name Quartz Health Solutions.

What is Quartz Health Plan Corporation?
Headquartered in Sauk City, Wisconsin, Quartz Health Solutions serves southern and central Wisconsin as well as part of surrounding states, including Minnesota, Illinois, and Iowa. Quartz is an administrator for various self-funded healthcare coverage plans and recently shifted into administrating the plans for several other local standard insurers, mainly HMO companies.
In the spring of 2017, Quartz partnered with Unity Health and Gundersen Health Plan, adding Physician’s Plus later that same year. All of these health plans, along with the Quartz ASO self-funded plans and Senior Preferred (which are Medicare plans offered through Gundersen), are now Quartz-branded affiliates. This change brings the Quartz name to the general public who are insured by one of those companies or seeking insurance from one of them.
As Quartz Health Benefit Plans Corporation affiliates, these regional insurance companies will continue to serve members as before, but under the new combined brand. The change is meant to offer members better access to health care providers and networks, including UW Health and UnityPoint Health-Meriter.
As of the time of this review, all agreements were completed between the companies. However, changes to branding and possible changes to health plans are still in progress, according to the Physician’s Plus website.
While all of the companies involved (except for Unity Health which now redirects to Quartz) will continue to maintain their own websites, the redirection of links between the sites has increased and will likely continue to increase. The Quartz website states that the process of integrating customer service resources is still in progress.

What health care coverage products are offered by Quartz Health Insurance Corporation?
As of the time of this review, Quartz still points to each individual health insurance company under its umbrella for plan listings and products. As an administrator, they provide services for a large array of comprehensive plans.
For this review, we will provide an overview of each health insurance company now under the Quartz brand and recommend further reading on that company and its medical insurance plans on our individual reviews of those companies.
Since Unity Health has fully re-branded to Quartz, we will provide a more detailed look at those plans.
Gundersen Health Plan offers HMO, PPO, and Medicare plans to members in parts of Wisconsin, Minnesota, and Iowa. They also offer Medicare Advantage through their Senior Preferred plans, which are HMO plans.
Physician’s Plus writes individual HMO plans in portions of Wisconsin. They also offer Medicare Advantage and group health insurance options.
Unity Health Quartz Branded Plans
Unity Health is a Wisconsin managed care insurer connected with UW Health. They are the only ones to have fully branded to Quartz and offer their plans through the Quartz website. Unity’s plans include a variety of individual HMO plans at all levels, Medicare Supplement, and Group health insurance.
Plans are offered at Catastrophic, Bronze, Silver, and Gold levels.
Catastrophic Plans
The Prime Catastrophic plan has a $7,350 individual and $14,700 family deductible, with all healthcare services covered in full once the deductible is met. The only covered service with this type of plan before meeting the deductible is preventative care. All other healthcare services are subject to the deductible.
Bronze Plans
There are five different Bronze plans, all with slight differences in coverage. Three HSA plans are offered, with individual deductibles of $5,000, $6,000, and $6,550.
The two higher deductible options feature 0% coinsurance once the deductible is met, while the lower $5,000 deductible option has 20% coinsurance after the deductible is met.
The two other Bronze plans are similar, with one offering more copay options. The Deductible $6,500 plan has 40% coinsurance after the deductible is met, with copays of $25 for primary care and prescriptions starting at the same amount. All other medical expenses and services are subject to the deductible.
The Bronze PCP copay $35 plan has a $6,650 individual deductible. As stated in the name, the primary care copay is $35, while there are also copays for specialist care and urgent care, both at $75. Prescriptions start at a $35 copay.
Silver Plans
Of the five Silver level plans, two offer an HSA. The first has a $3,000 individual deductible with a 30% coinsurance after the deductible is met. The second has a higher deductible at $5,050 but covers all medical costs at 100% after it is met.
The remaining three are PCP Copay plans. The highest deductible option is the $7,100 individual plan, which has a primary care copay of $75 and a specialist copay of $150. Prescriptions start at $15. The second option has a $5,000 individual deductible with a $35 primary care copay and a $75 specialist copay. Again, prescriptions start at $15.
Finally, the lowest deductible is the $3,000 individual, with a primary care copay at $30. Specialist visits have a $65 copay. While prescription copays again start at $15, there is a pharmacy deductible of $500 with this plan.
Gold Plans
At the Gold level, there is only one HSA plan, which has an individual deductible of $1,800 and a 10% coinsurance when that is met. The remaining Gold plans are copay-based. The highest deductible option is $3,500 for an individual.
This plan has copays of $30 for primary care and $60 for specialists. The next highest deductible option is $2,900 for an individual, and this plan has the same copays at $30 for primary care and $60 for specialists. Both of these plans have prescription coverage starting at $10.
The final two plans both have a $20 copay for primary care, with deductible options of either $1,700 individual or $1,400 individual. The plan with the higher deductible has a higher specialist copay at $70 vs. $50, but prescriptions start at $5 vs. $10.
Self-Funded Plans
Quartz ASO administers self-funded health plans that provide employers the option to provide employee benefits with access to their partner networks.
Are the company’s rates affordable?
Rates for Quartz plans will vary depending on the affiliate company through which you obtain your coverage. Quoting directly from the Quartz website, we could return rates for Unity Health plans that are now Quartz branded.
The least expensive quote we could find for a 30-year-old single person in Dane County, Wisconsin, was the Catastrophic Plan at $245.84 per month. From there, the plans jump well into the $300 per month range and above, with the Prime Gold Standard the most expensive at $488.65 a month.
While we are unable to compare these rates to our usual sample group, the monthly premium is a little higher than what we have seen from other Wisconsin HMOs.

How are claims handled?
Currently, each individual affiliate in the Quartz family of companies appears to continue handling its own internal claims. This may differ from Unity, which is now fully Quartz branded, but there is no information on the website regarding this.
Are the company’s ratings and consumer reviews positive?
Looking up reviews and ratings for a company like Quartz can lead to some confusion since there are several underlying companies, each with its own reputation.
As the company moves forward with the changes, we will likely see the ratings become more focused on Quartz. In the meantime, however, it is a good idea to look at the individual reputation of the underlying company.
We could not find a Better Business Bureau (BBB) page for Quartz, nor did we locate any specific reviews of this company. It will likely require more time before we see direct consumer reviews since their move into individual insurance is recent.
We did find at least one comment in Google reviews from a Unity Health customer stating that they have had a negative experience since the Quartz change. Other reviews have cited premium increases with lower coverage. There are a few positive reviews of Unity, including one since the changeover.

What’s the bottom line?
Quartz is new to the individual health insurance plan market, but their affiliated companies are not, so there is plenty of history behind them.
Since it will take some time for the dust to settle and all of the changes to be completed, those looking to switch insurance companies may want to wait it out. Until Quartz develops a reputation of its own, we recommend looking at the affiliate company directly for further information.
